Important Notes
1. Fees rise April 14, 2012. If you submit this form and payment on or before April 13,
2012 you will not have to pay the increased fee.
2. You are not fully registered with the NNISC until payment is received by the NNISC Executive Committee. Late registration does not guarantee placement on a team and will be on a best effort basis.
3. This is the registration process for the NNISC only; you will be provided with instructions at a later date on how to register as a player within the NCISL (to complete the EODSA requirements). |

Payment Information
